Howland Public Library is an official Passport Acceptance Facility.

You must apply in person at an acceptance facility if any of the following are true:

  1. You have never been issued a U.S. passport or

  2. You were under age 16 when your previous passport was issued or

  3. Your most recent U.S. passport was issued more than 15 years ago or

  4. Your most recent U.S. passport was lost/stolen or

  5. Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change.

The Howland Public Library is located at 313 Main Street, Beacon, NY 12508.

  • You will need to bring
    -proof of ID, such as a driver’s license
    -proof of U.S. citizenship, such as an original or certified birth or naturalization certificate, or a previous U.S. passport
    -the appropriate fees.

    Please note, you cannot use the same document as both proof of citizenship and ID.

  • Age 15 or under: Both parents (or the child’s legal guardians) must appear with the child. Click here for more information if both parents cannot be present. Evidence of the child’s U.S. citizenship, such as an original birth certificate; Evidence of the child’s relationship to parents (original birth certificate); and Parents’ government-issued ID (such as driver’s license)

    For minors aged 16 or 17 without their own Driver's License (not Learner's Permit) we recommend that both parents come and provide ID to prevent processing delays.   • Approximately 30 minutes per appointment.

  • Yes, we take passport photos. The charge is $10.00.

  • 1. Execution Fee: $35/per new application

    Payment to Howland Public Library can be in the form of: money orders, personal checks, or credit cards.

    *Payments to US Department of State must be a check or money order (no starter checks).

  • We cannot do renewals. Renewals must be completed and mailed on your own. We can offer help if you have questions, but cannot accept or mail your application.

    However, if your passport was issued when you were 18 years old or older AND was issued more than 15 years ago you are eligible for a new passport and can make an appointment with us.

HOW TO APPLY FOR A PASSPORT

Follow the instructions below exactly.

1. Make an appointment online.

2. Complete and print form before your appointment.

Helpful Tip:
Enter the information online and then print Form DS-11. OR print a blank form DS-11 and complete it by hand in black ink. Do not sign the DS-11 form until the Passport Agent instructs you to do so.

3. Submit your completed application form and the following additional documents during your appointment.

A photocopy of both the front and back of both citizenship and identity evidence is required! You can make a copy at the library.

Proof of U.S. Citizenship (one of the following):

  • U.S. birth certificate or certified copy

  • Previous U.S. passport (can be fully-valid or expired, but must be undamaged)

  • Certificate of Naturalization

  • Certificate of Citizenship

  • Certificate of a U.S. Birth Abroad

    Proof of Identity (one of the following; originals only):* 

  • Valid in-state driver's license (not temporary or learner's permit)

  • Valid out-of-state driver's license (not temporary or learner's permit.) If you have an out-of-state license, you must provide an additional proof of identity

  • Previous U.S. passport book or passport card (can be fully valid or expired, but must be undamaged)

  • Certificate of Naturalization with an identifiable photo attached

  • Certificate of Citizenship with an identifiable photo attached

  • Government employee identification card (federal, state, county, municipal)

  • Official U.S. military or military dependent identification card√ Current (valid) foreign passport

4. Pay the Applicable Fees during your appointment.

Two payments are required:

  • Payment 1:
    Payment to the U.S. Department of State for the cost of your passport. Payment to the U.S. Department of State must be in the form of personal CHECK or MONEY ORDER (no starter checks accepted).

    • Fees are as follows:
      Adult - $130
      Minor - $100
      Expedite Fee - $60

  • Payment 2:
    Payment to Howland Public Library for the application fee and/or photo. Payments to the Library can be by check, money order, or credit card.

      Library Passport Execution Fee: $35
      Photo (if needed): $10
